The upper part of the Shipper Certificate is an official statement required by the DOT to accompany shipments of ECDs.
The body of the form allows you to inform us of your Company Data and the status of the detectors which you have sent in for service. For first time customers please include a copy of the purchase order or a business card so that the company information will be input correctly.
Indicate the turnaround requested. If this part is left blank we will service the ECD in a standard turnaround 7 - 10 days. Urgent and Sameday turnaround service is not always available and require pre-arrangement. Please contact CJ Bruyn & Co if you wish to request an Urgent or Sameday turnaround.
Complete the Ship to and Bill to information and as much as the ECD identification information and symptoms as you can. Also indicate preferred method of return shipping. If this is left blank we will ship 2 day service. See Shipping information for more information.
A Confirmation of Receipt will be faxed with 24 hours of the ECD arriving at CJ Bruyn & Co. Indicated on the form will be the expected ship date. We will ship as close to this date as possible. If you requested a quote this will be faxed after the technician has evaluated the ECD and determined the service required to bring it back to proper operating condition.
Each serviced ECD will be returned with a Shipper Certificate with the company information you provided filled out. Please check it for accuracy and make any needed changes.
Each serviced ECD will also be tested for removable activity. A Certificate of Leak-Test will accompany the returned ECD. Keep this for your records.
All ECD's are cleaned and tested. A New Source will be installed if tests indicate this to be necessary.
